Pregnancy
AvoidSt. John's Wort has hormone-modulating activity and significantly interacts with many medications (it induces liver enzymes that metabolize a huge range of drugs) — generally avoided in pregnancy, and any use requires medical coordination given the interaction risk alone.
Breastfeeding
AvoidSome case reports describe infant drowsiness/colic when the nursing parent used St. John's Wort; combined with its extensive drug-interaction profile, it's generally avoided while nursing.
Children
AvoidNot typically used in children given its significant drug-interaction profile and hormone-modulating activity.
Interactions & contraindications
Antidepressants (SSRIs, SNRIs, MAOIs)
Combined use raises the risk of serotonin syndrome.
Hormonal birth control
Induces liver enzymes that break down oestrogen, reducing contraceptive effectiveness.
Warfarin and other blood thinners
Speeds up drug metabolism, reducing effectiveness and clotting protection.
Immunosuppressants (cyclosporine, tacrolimus)
Can lower drug levels below the therapeutic range — a serious risk for transplant patients.
Triptans and other migraine medications
May increase serotonergic side effects.
