Hand Foot Mouth

Hand, foot, and mouth disease (HFMD) is a contagious viral syndrome caused primarily by enterovirus 71 (EV-A71) and coxsackievirus A16 (CV-A16), with coxsackievirus A6 increasingly responsible for more severe presentations. The virus replicates in the oropharyngeal and intestinal mucosa after faecal-oral or respiratory droplet transmission, then disseminates haematogenously to infect keratinocytes in the palms, soles, and oral mucosa — producing the distinctive triad of oral ulcers, vesicular rash on hands and feet, and fever. Oral lesions typically begin as red macules on the buccal mucosa, tongue, and palate that ulcerate rapidly, causing significant pain and refusal to eat or drink. Cutaneous lesions are typically 3–7 mm grey-white vesicles on an erythematous base, concentrated on the palms, soles, and perioral area — in coxsackievirus A6 infection they may be larger, more widespread, and affect the trunk and buttocks (atypical HFMD). EV-A71 in particular is associated with rare but serious neurological complications (viral meningitis, rhombencephalitis, acute flaccid paralysis) — concentrated surveillance in Asian outbreaks has defined these risk patterns. Nail shedding (onychomadesis) — complete or partial nail loss beginning 3–8 weeks after resolution — occurs in 15–50% of cases, alarming but self-resolving. The illness is self-limiting in 7–10 days. Holistic support focuses on oral soothing (chamomile and licorice root gargles or frozen herbal pops that simultaneously soothe mucosa and provide hydration), immune support (elderberry, zinc, astragalus), and barrier repair through calendula topical preparations for skin lesions.

Elderberry Syrup (antiviral)

Sambucus nigra

Hand, foot and mouth disease (HFMD) is caused by Coxsackievirus A16 or Enterovirus 71 — both are enteroviruses against which elderberry anthocyanins have demonstrated antiviral activity.

Preparation:

  • Elderberry syrup — commercial or homemade (honey for 1y+ only).
  • Given at first sign of illness.

Dosage:

  • Children (1–3y): 1 tsp, 4× daily.
  • Children (4–12y): 1 tbsp, 4× daily.
  • Teens: 2 tbsp, 4× daily.

Avoid if:

  • Raw elderberries toxic
  • No honey under 12 months

European Herbalism

Manuka Honey (oral sores)

Leptospermum scoparium (nectar)

Applied directly to painful oral sores from HFMD — manuka honey provides direct antimicrobial protection, reduces viral replication in mucosal tissue, and coats the ulcers to reduce pain on swallowing.

Preparation:

  • Apply ½ tsp Manuka honey directly to oral sores with a clean finger or spoon.
  • For toddlers: place on dummy/pacifier momentarily.
  • Give 3–4× daily.

Dosage:

  • Children (1y+): ½ tsp applied to oral sores, 4× daily.
  • Also effective as 1 tsp in warm water to gargle or rinse (for children who can rinse).

Avoid if:

  • Never for under 12 months — infant botulism risk

Maori / Western Herbalism

Chamomile (mouth rinse)

Matricaria chamomilla

Chamomile mouth rinse soothes the painful oral blisters and ulcers of HFMD — anti-inflammatory and mild antiseptic.

Preparation:

  • Strong chamomile tea (2 tsp dried flowers in 250 ml boiled water, 10 min).
  • Cool.
  • For younger children: dip a gauze in cooled chamomile tea and gently dab mouth ulcers.

Dosage:

  • Children (2–5y): dab sores with chamomile gauze, 4× daily.
  • Children (5+): rinse mouth gently for 30 sec, 4× daily.

Avoid if:

  • Asteraceae allergy

European Folk

Cold Smooth Foods (pain management)

HFMD oral sores make eating very painful — cold, smooth foods are the most accepted and least painful during the acute phase.

Preparation:

  • Offer: cold yoghurt, cold mashed banana, ice cream (small amounts), fruit popsicles, chilled cucumber.
  • Avoid: acidic foods (worsen ulcer pain), crunchy foods (abrasion), hot foods.

Dosage:

  • As much as the child will accept.
  • Prioritise hydration above nutrition in the first 2–3 days.

Universal

Manuka Honey & Elderberry Popsicles

Combines pain management (cold), antiviral (elderberry) and wound healing (honey) in a format children will readily accept.

Preparation:

  • Mix 2 tbsp elderberry syrup + 1 tsp Manuka honey + diluted pear juice.
  • Pour into popsicle moulds.
  • Freeze.
  • Offer throughout the day.

Dosage:

  • 2–4 popsicles daily during acute HFMD for children 1y+ only (honey).

Western Herbalism / Modern

Coconut Water Hydration

HFMD children often refuse fluids due to oral pain — coconut water's mild sweetness and isotonic electrolytes are more accepted than plain water.

Preparation:

  • Offer room-temperature coconut water (not cold — ice-cold can cause discomfort).
  • Small sips every 10–15 min.

Dosage:

  • Small sips throughout the day — hydration is the primary concern in HFMD.
  • Hospitalisation occurs due to dehydration, not the rash.

Hand, Foot & Mouth Disease Recovery Protocol

Western Integrative / Universal

  • HFMD is a self-limiting viral illness (Coxsackievirus A16, Enterovirus 71) primarily affecting children under 10.
  • The primary treatment goals are maintaining hydration, reducing oral sore pain to allow fluid intake, and supporting antiviral immune response.
  • Most cases resolve in 7–10 days.

Foods:

  • Priority 1: Hydration — coconut water, diluted pear juice, cold popsicles
  • Priority 2: Soft cold foods — cold yoghurt, banana, avocado, popsicles
  • Elderberry syrup 4× daily throughout illness
  • Manuka honey directly on oral sores 4× daily (1y+ only)
  • Chamomile mouth rinse / gauze dab 4× daily
  • Avoid: acidic foods (citrus, tomato, vinegar — worsen sore pain)
  • Avoid: hot, crunchy, or spicy foods throughout acute phase

Timing:

  • Day 1–3 (acute): Elderberry popsicle + coconut water sips + Manuka honey on sores — hydration-first approach when eating is refused due to pain
  • Day 3–5 (improving): Cold mashed banana + yoghurt + diluted pear-elderberry juice + warm chamomile tea (sipped gently)
  • Day 5–7 (recovering): Soft rice porridge with bone broth + avocado + return to soft normal foods as sores resolve

  • Serve cool but not ice cold.

Dosage:

  • 120–200 ml, 3× daily during HFMD.
