Kids Dairy Intolerance

Lactose intolerance in children results from relative deficiency of intestinal lactase — the brush border enzyme secreted by small intestinal villous enterocytes that cleaves the disaccharide lactose (milk sugar) into absorbable monosaccharides glucose and galactose. Undigested lactose passes unabsorbed into the colon, where resident bacteria ferment it to short-chain fatty acids, hydrogen gas, CO2, and methane — producing the characteristic bloating, abdominal cramps, flatulence, and osmotic diarrhoea. Primary lactase persistence (the genetic adaptation allowing continued high lactase production into adulthood) evolved in populations with a long history of cattle domestication; globally, 65–70% of adults are lactase non-persistent — making lactose intolerance a species-normal variant rather than a disorder. In children, secondary lactose intolerance frequently follows gastroenteritis: rotavirus, norovirus, and Giardia damage the villous brush border, transiently depleting lactase (which is located at the villous tip — most vulnerable to injury) for weeks after infection resolves. Milk protein allergy (whey and casein IgE or non-IgE mediated reactions) is distinct from lactose intolerance: it causes immune-mediated symptoms (urticaria, eczema, vomiting, anaphylaxis) rather than fermentation-derived symptoms, and affects the infant immune system differently.
