Kids Wound Care

Wound management in children encompasses the full spectrum from minor abrasions ('grazes' — superficial scrapes removing only the outer epidermis) to lacerations (full-thickness skin cuts), puncture wounds, and bite injuries. Normal wound healing follows three overlapping phases: the inflammatory phase (hours 1–72) — haemostasis via platelet aggregation and fibrin clot formation, followed by neutrophil infiltration clearing bacteria and debris, then macrophage arrival coordinating the repair response through growth factor secretion (VEGF, PDGF, TGF-β). The proliferative phase (days 3–21) — fibroblast migration and collagen deposition rebuilding the dermis; angiogenesis restoring blood supply; keratinocyte migration from wound edges resurfacing the wound. Remodelling (months to years) — collagen crosslinking and reorganisation improving tensile strength, gradual scar maturation. Children heal faster than adults (higher fibroblast activity and growth factor concentrations, better angiogenic response) but are more prone to keloid and hypertrophic scarring — particularly in high-tension skin areas. Tetanus risk must be assessed for contaminated deep wounds: children should be up to date with tetanus toxoid vaccination; contaminated wounds in under-vaccinated children require tetanus prophylaxis consideration. Bite wounds (human and animal) carry high infection risk from polymicrobial oral flora (Pasteurella multocida from cat and dog bites in particular — aggressive subcutaneous spread). Holistic wound management: clean irrigation with saline (the most important infection prevention step — 'the solution to pollution is dilution'); manuka honey dressings (UMF-grade — documented wound healing acceleration and antimicrobial in multiple clinical studies); calendula for epithelial regeneration; aloe vera; and tea tree oil (diluted) as topical antimicrobial.
