Night Terrors

Night terrors (sleep terrors) are a non-REM parasomnia — episodes of sudden partial arousal from deep slow-wave sleep (stage N3/N4) — occurring in 1–6% of children, predominantly between ages 3 and 8. They arise during the first third of the night (when slow-wave sleep is densest), typically 1–3 hours after sleep onset, as the brain attempts to transition from deep N3 sleep to lighter stages but gets 'stuck' in an intermediate state of simultaneously high arousal activation and deep sleep inhibitory tone — a state of incomplete arousal. The child appears to be awake and terrified — screaming, thrashing, with elevated heart rate, dilated pupils, flushing, and sweating (autonomic storm from limbic system activation without cortical override) — but is actually deeply asleep and cannot be fully roused. Crucially, the child does not perceive the episode consciously and retains no memory of it the following morning; the event is far more distressing for parents than for the child. Excessive slow-wave sleep 'pressure' (depth) is the key predisposing factor: sleep deprivation (either insufficient total sleep or irregular sleep schedule) dramatically increases slow-wave sleep rebound on subsequent nights, precipitating night terror episodes — making paradoxically maintaining adequate sleep duration the central treatment. Fever, stress, and sleep disruption are common triggers. The condition is strongly familial (positive family history in 80% of cases), suggesting genetic variation in arousal threshold.
